Application of botulinum toxin for the treatment of vascular distress in local flaps in the island

Abstract

The use of botulinum toxin for purposes other than aesthetic use has been documented. Botulinum toxin has not only shown action by blocking the release of acetylcholine, it also blocks the synaptic release of vesicles containing norepinephrine causing a sustained sympathetic blockade, altering the expression of vascular mediators, promoting angiogenesis and reducing pain. The vasodilatory and pro-angiogenesis effect has been studied both in animal models and in humans to prevent secondary ischemia in free flaps, improve signs of local flap suffering, as a rescue measure in limbs with suffering, improvement of perfusion in patients with raynaud, among other uses. Therefore, we have used the use of botulinum toxin in locoregional island flaps with signs of suffering, finding a decrease in the progression of these signs as well as a rapid and effective improvement in the survival of these flaps.
